POPULATION WE SERVE: Single men & women who are homeless and who live with multiple issues: (i.e.) chemical dependency, mental illness, domestic violence, medical disabilities, and sometimes life-threatening illnesses
DESCRIPTION OF PROGRAM: We expect the participants to identify their own goals and assist them in developing and implementing steps toward their goals which may involve referrals to other agency/service providers.
PROGRAM GOALS:
1. Assisting a person from being homeless to greater self-sufficiency and permanent housing.
2. Connecting participants with a productive network of community services.
3. Building & nurturing self-confidence through learning life skills (i.e.): self-care, learning to discover healthy relationships, addictions and/or mental health services, developing employment skills, securing employment, continuing education, reconnecting with family, money management and following up on critical needs and services.

HOW DO PARTICIPANTS EXPERIENCE “SUCCESS”?
When people learn from their past and learn new life skills such as:
• Financial responsibility
• Finding & maintaining employment
• Going back to school
• Reuniting with family in healthy ways
• Becoming more mentally/emotionally stable
• Embracing recovery from various addictions
• Securing permanent housing
Basically learning to map out their life with attainable goals a day at a time and coming to realize that the process in how you do this is even more important than the goal itself!
VOLUNTEERS COULD BE USED TO:
• Assist with our House Groups on various life-skill topics
• Drive for special events
DONATIONS NEEDED YEAR ROUND! Always can use:
* paper products *cleaning supplies
* hygiene products * towels (old & new)
* Bus tokens & Passes * Newspaper subscriptions
* bedding for single beds (blankets, pillows, mattress covers)
*Phone cards
• For those moving into their apartment:
Cleaning/household care kit and any kitchen wear
• On occasion a TV and some household furniture to furnish our Transitional houses
PHYSICAL BOUNDARIES: Most of our Transitional Housing sites are in walking distance from each other and have either single or 2 person bedrooms. They are near the bus routes.
OTHER INFORMATION:
• TH programs range from a few months up to 2 years.
• All TH sites have a weekly Group meeting with at least one service coordinator present
• Participants meet regularly with their Service Coordinator. New participants are expected to meet weekly.
• People in our programs are called participants.
• Participants are expected to pay a monthly participation fee.
