Outcomes Success

Throughout 2003 and 2004, the Service Coordination staff have worked with Outcome Specialist, Matt Mendenhall, MSW, to identify and measure the incremental steps that program participants take toward achieving the long-term goal of stable housing and financial security.

First, JLCS believes that basic needs must be met before an individual can make any decisions or life changes.

  • Food
  • Clothing
  • Shelter

Second, JLCS believes supportive resources must be in place, as an individual begins to make life changes.

  • Medical Assistance
  • Substance Abuse Counseling
  • Social Security Advocacy
  • Mental Health Counseling

Third, once these basic needs and resources are in place it is possible for an individual to examine the barriers for creating real change.

  • Service Coordinators work with participants to examine the barriers for success and to create concrete for overcoming these barriers.
    – e.g. substance abuse recovery meetings
    e.g. parenting skills
    e.g. mental health counseling

Fourth, as program participants work toward achieving their  goals, the Service Coordinator begins to coach the individual to strengthen these essential linkages, and to develop lasting supportive connections in the community.

  • Ongoing supportive AA groups
  • Developing friendships
  • Understanding community resources

Fifth, the program participant begins to achieve the long-term goal of lasting housing stability, income stability, and skill development.

  • Employment
  • Disability Social Security Income
  • Achieving appropriate education
  • Affordable housing
